     § 912.  Applications for licenses and permits.
        Every application for a license or a permit shall:
            (1)  Be in writing on a form supplied by the commission.
            (2)  Be verified by the applicant.
            (3)  Set forth such information and have attached thereto
        such photographs and other exhibits as are required by this
        subpart, the rules and regulations promulgated under this
        subpart and the form of application.
