§ 158.106. FORMS FOR INCOME WITHHOLDING. (a) The Title IV-D agency shall prescribe forms as required by federal law in a standard format entitled order or notice to withhold income for child support. (b) The Title IV-D agency shall make the appropriate forms available to obligors, obligees, domestic relations offices, friends of the court, and private attorneys. (c) The Title IV-D agency may prescribe additional forms for the efficient collection of child support and to promote the administration of justice for all parties. (d) The forms prescribed by the Title IV-D agency under this section may be used to request voluntary withholding under Section 158.011. Added by Acts 1995, 74th Leg., ch. 20, § 1, eff.
