Indiana Code - Motor Vehicles - Title 9, Section 9-14-5-1

Qualification for issuance of placard

Sec. 1. The bureau shall issue a placard to the following:
(1) An individual of any age who:
(A) has a temporary or permanent physical disability that
requires the use of a wheelchair, a walker, braces, or
crutches;
(B) has temporarily or permanently lost the use of one (1) or
both legs;
(C) is certified to be severely restricted in mobility, either
temporarily or permanently, due to a pulmonary or
cardiovascular disability, arthritic condition, or orthopedic
or neurological impairment, by:
(i) a physician having an unlimited license to practice
medicine;
(ii) a physician who is a commissioned medical officer of
the armed forces of the United States or of the United
States Public Health Service;
(iii) a physician who is a medical officer of the United
States Department of Veterans Affairs;
(iv) a chiropractor licensed under IC 25-10-1; or
(v) a podiatrist licensed under IC 25-29-1; or
(D) is certified by an optometrist or ophthalmologist
licensed to practice in Indiana to be blind or visually
impaired.
(2) Any corporation, limited liability company, partnership,
unincorporated association, and any legal successor of the
corporation, limited liability company, partnership, or
association, empowered by the state or a political subdivision
to operate programs, including the provision of transportation,
or facilities for persons with physical disabilities.
