Physician certification form
Sec. 2. The certification made by a physician, an optometrist, or
an ophthalmologist under section 1(1)(C) or 1(1)(D) of this chapter
must be on a form prescribed by the state board of accounts and must
state the expected duration of the condition that severely restricts the
individual's mobility.
