Oath and affirmation; signature
Sec. 3. (a) The form described under section 2 of this chapter
must include the following printed statement:
"I swear or affirm that the information I have entered on this
form is correct. I understand that making a false statement on
this form may constitute the crime of perjury.".
(b) The person applying for the certificate of title must sign the
form directly below the printed statement.
