Change of residence or name; notification
Sec. 4. If:
(1) an individual holding a license or permit issued under this
article changes the address shown on the license or permit
application; or
(2) the name of a licensee or permittee is changed by marriage
or otherwise;
the licensee or permittee shall immediately notify the bureau in
writing of the licensee's or permittee's old and new address or of the
former name and new name and the number of the license or permit
held by the licensee or permittee.
