Lost or destroyed licenses
Sec. 1. If a permit or license issued under this article, except
section 2 of this chapter, is lost or destroyed, the individual to whom
the permit or license was issued may obtain a replacement if the
individual does the following:
(1) Furnishes proof satisfactory to the bureau that the permit or
license was lost or destroyed.
(2) Pays the required fee for a replacement permit or license
under IC 9-29-9.
