Any person operating a body piercing facility licensed under ORS 690.520 shall:
(1) Provide to all customers a written statement approved by the Oregon Health Licensing Agency that advises the customer of risks or dangers involved in the procedure and all complications that may occur;
(2) Post in public view in the body piercing facility a disclosure statement established by the agency by rule; and
(3) Post in public view in the body piercing facility a notice containing the address of the agency and the procedure for filing a complaint as established by the agency by rule. [1995 c.562 §4; 2001 c.104 §272; 2005 c.648 §84]
