Section 45-49-43.02 - Application - Requirements.

Applicants for license under this part shall file with the license commission a sworn application in writing, in duplicate, on a form to be furnished by the license commission which shall give all of the following information:

(1) Name and physical description of the applicant.

(2) Date of birth and drivers license number of the applicant.

(3) Full local address of the applicant, and the length of time the applicant has resided within the county.

(4) If employed, the name and address of the employer, together with credentials establishing the employer-employee relationship.

(5) A brief description of the nature of the gold or silver buying to be done.

(6) The length of time for which the right to do gold or silver buying is desired and the location where such buying is to be done.

(7) A photograph of the applicant, taken within 60 days immediately prior to the date of filing the application, which picture shall be not smaller than two inches by two inches, showing the head and shoulders of the applicant in a clear and distinguishing manner.

(8) The fingerprints of the applicant, and the names of at least two reliable property owners of the county who will certify as to the applicant's good character and business respectability or, in lieu of the names of such references, such other available evidence as to the good character and business responsibility of the applicant, as shall enable an investigator to properly evaluate such character and business responsibility.

(9) A statement as to whether or not the applicant has been convicted of any crime, misdemeanor, or violation of any municipal criminal ordinance, in any state, the nature of the offense, and the punishment or penalty assessed therefor.

(10) The names, local addresses, drivers license numbers, and physical descriptions of all employees of the applicant.
