Sec. 2. A person applying for a certificate of title for a vehicle
must submit an application on a form furnished by the bureau that
contains the following information:
(1) A full description of the vehicle.
(2) A statement of the person's title and of any lien or
encumbrance on the vehicle.
(3) Other information the bureau requires.
