Approval of forms by Labor Commissioner. The Labor Commissioner shall approve the form of the receipts given, records kept and applications provided by a licensee and the form of the contract used by a licensee for its services to applicants for employment. Each licensee shall provide the necessary blanks and other supplies at his own expense.
