Petition: Filing, contents and supplemental documents. A petition filed pursuant to NRS 433A.703 must:
1. Be filed with the district court of the county where the petitioner resides or, if the petitionerís admission was involuntary, with the district court which ordered the admission.
2. Set forth the facts bringing the petitioner within the purview of that section.
3. Be accompanied by the affidavit of a psychiatrist, psychologist or physician qualified in the field of psychiatric mental health who has examined the petitioner. The affidavit must:
(a) Summarize the professional qualifications of the affiant;
(b) Set forth the date or dates on which he examined the petitioner; and
(c) State that, in the opinion of the affiant, the petitioner has recovered or his illness is in substantial remission.
