Application; fees. [Effective on the date of the repeal of the federal law requiring each state to establish procedures for withholding, suspending and restricting the professional, occupational and recreational licenses for child support arrearages and for noncompliance with certain processes relating to paternity or child support proceedings.] An applicant for examination for a license to practice Oriental medicine or any branch thereof, shall:
1. Submit an application to the Board on forms provided by the Board;
2. Submit satisfactory evidence that he is 21 years or older and meets the appropriate educational requirements;
3. Submit with the application a complete set of his fingerprints which the Board may forward to the Central Repository for Nevada Records of Criminal History for submission to the Federal Bureau of Investigation for its report;
4. Pay a fee established by the Board of not more than $1,000; and
5. Pay any fees required by the Board for an investigation of the applicant or for the services of a translator, if the translator is required to enable the applicant to take the examination.
