The notice of class action states that, as a condition of the settlement of Campbell v. State Farm, State Farm had deposited $11,250,000 into trust to be used to pay members of the class. According to the notice of class action, a class member was required to submit a proof of claim questionnaire and certain other materials in order to be eligible to receive a payment.
